Role and Responsibilities:

• Demonstrates knowledge of home health and competency in discipline specific patient care skills such as activities of daily living, energy conservation, UE strengthening, and pain management.
• Performs home health occupational therapy services in accordance with physician’s orders and agency policies.
• Performs patient comprehensive assessments, as assigned, and collaborates with patient multi-disciplinary care team (SN, PT, ST, MSW and paraprofessionals) and other health care professionals to develop/modify and implement an individualized plan of care as per physician orders that ensures quality, proper discharge planning and achieves desired outcomes and goals.
• Supervise and educate licensed Occupational Therapy Assistants and Home Health Aides
• Participates in clinical outcomes monitoring, follow up and Quality Assurance Performance Improvement initiatives.
• Utilizes accepted company teaching materials in patient/family education.
• Regularly communicates patient progress to the clinical leadership and the patient’s physician.
• Completes documentation timely, accurately, and at the point of care, according to industry standards, and guidelines.

Required:

• Current Illinois license to practice as an Occupational Therapist.
• One (1+) year of clinical experience as an Occupational Therapist.
• Current CPR certification.
• Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online training programs.
