Susan Wrzeszcz

Susan serves as the Vice President of Clinical Operations for Empower Home Health. In her role, she oversees all clinical operations, ensuring the highest quality of care and outcomes through managing clinical compliance. Susan holds a Bachelor of Science in Nursing from Elmhurst University and has 33 years of clinical practice experience in various healthcare settings, including hospital intensive care and critical care. Her extensive background as a field clinician in home health care for over 30 years equips her with the skills and expertise to support Empower Home Health clinicians in meeting the goals of each individual patient.

Throughout her career, Susan has witnessed significant changes in the scope of home health, bringing valuable perspective to the team. Her experience in Centers for Medicare and Medicaid Services and The Department of Health and Human Services guidelines allows her to establish clinical protocols and best practices that meet the needs of the patient.

Susan is a dedicated advocate for patients, going the extra mile to ensure they receive the best possible care. She resides in West Dundee with her husband, Mike, and takes pride in raising three amazing children, Kristen, Lauren, and Ryan. Kristen and Lauren have followed in their mother’s footsteps, serving the community as nurses both in the hospital and at Empower. A fun fact: one of her daughters named Empower!

Evie Gagliano

Evie is the Vice President of Therapy Services at Empower Home Health. Leading the therapy team, she oversees all physical, occupational, and speech therapy clinicians. Evie’s innovative thinking and commitment to out-of-the-box solutions have been instrumental in developing treatment protocols that yield the best outcomes and maximize patient independence. Evie is a physical therapist by trade, having earned her Master’s and Doctoral Degree in Physical Therapy from Washington University in St. Louis, MO.

With 14 years of experience, she specializes in developing specialized exercise programs for the home environment, focusing on geriatric populations and those with chronic illnesses. She is certified as an Exercise Expert for the Aging Adult through the geriatric section of the American Physical Therapy Association (APTA) and is BIG Certified through LSVT, providing evidence-based therapy treatments for patients with Parkinson’s disease and other conditions.

Evie’s experience across various patient diagnoses makes her a valuable resource for therapists in the field. She has a special knack for connecting with patients, ensuring exceptional care. Evie resides in Lake Zurich with her two children and their beloved dog, Wesley. She enjoys spending quality time with her family and traveling, creating lifelong memories together.

Stephanie Saxton

Stephanie is the Agency Supervisor and Director of Clinical Care at Empower Home Health. She oversees the daily clinical services provided by the team and is known for her meticulous attention to detail and extensive knowledge of the ever-changing healthcare landscape. Her expertise is particularly valuable for the company, as she specializes in clinical education, quality assessment of patient care, and performance improvement, leading to high patient satisfaction.

Stephanie is a Registered Nurse with a Bachelor of Science in Nursing from Western Illinois University, graduating at the top of her class. With 10 years of experience in home health, Stephanie began as a field clinician, treating complex and acute patients in the home environment. This hands-on experience in the field gives Stephanie a unique perspective when working with our clinicians, providing training, troubleshooting, and support with great understanding and empathy.

Stephanie is deeply passionate about providing excellent patient care to those served by Empower Home Health. She resides in Crystal Lake with her family, which includes her husband, Mike, and her two children, Madison and CJ. In her free time, Stephanie enjoys the company of her family and friends and is always up for a good DIY project, especially if it involves creating something special for her littles.

Brittany Gamss

Brittany serves as the Administrator for Empower Home Health. Her role is to provide crucial support for the business staff and clinicians, ensuring regulatory compliance and patient advocacy. Despite her non-clinical capacity, Brittany plays a key role in maintaining the highest level of customer service and patient satisfaction. With 14 years of experience in overseeing home-based care, Brittany has worked in various departments, starting as a marketing and events coordinator and transitional care coordinator. She has built her knowledge of business analytics and regulatory compliance over the years.

Brittany holds a Bachelor of Arts in Communication Studies from Eastern Illinois University and completed her Master of Arts at Ashford University in Iowa. Her passion lies in helping families navigate the complex post-acute healthcare landscape, making her a valuable resource for patients, families, and staff. Brittany resides in Huntley, IL, with her family, including her husband, Ryan, of 8 years, her two children, Claire and Myla, and their pup Graham. In her free time, Brittany enjoys spending quality time with her family, exploring new places near and far, and watching movies with her hubs.

Geoffrey Beck

Director of Transitional Care

Geoffrey serves as the Director of Transitional Care at Empower Home Health, bringing a unique blend of expertise to the team. With a Bachelor’s degree in psychology and a Master’s degree in teaching, Geoffrey’s primary focus and passion lie in educating patients as they navigate the healthcare system. His mission is to empower patients with the knowledge and confidence to make informed decisions about their care.

Geoffrey’s journey in home health spans a decade, starting in marketing in the Rockford, IL market. His extensive experience has allowed him to develop a deep understanding of the industry, giving him valuable insights into the challenges and opportunities of transitional care.

While Geoffrey may not hold specific certifications, his commitment to patient education and his experience in the field make him a valuable resource for patients and the Empower Home Health team. His patient-centered approach is driven by his desire to ensure that every individual is well-informed and supported during their healthcare transitions.

Geoffrey resides in Shirland, Illinois, with his wife Jessica and their two children, Braydon and Bryson. Beyond his professional life, he finds great fulfillment in family activities. Geoffrey’s personal interests include a strong passion for fitness and nutrition, a thirst for learning new skills, and a love for spending time in nature. As a “country boy” at heart, he enjoys the company of his three little dogs and tending to a lively herd of chickens. With a deep connection to both the healthcare field and the natural world, Geoffrey brings a unique perspective to the Empower Home Health team, dedicated to educating and empowering patients on their healthcare journeys.
